The Cancer Buddy Referral Service is a way to connect people who have similar circumstances in dealing with cancer and would like someone to talk to. The information on the Cancer Buddy Referral Service Questionnaireis completely confidential and will not be shared with anyone.
Participation is completely voluntary and names will not be given to anyone until permission is
granted by you on the form.

Help us develop a program where people with the same cancer diagnosis, who want to talk
with others facing a similar journey, can connect. We are asking interested people to
complete our Cancer Buddy Referral Service Questionnaire. When we get enough people to
respond who would like to use this service, we will then try to match up those who have
expressed an interest in sharing. The Buddy Program has been used by the Wellness
Community in the Bay Area for many years with great success.
